Instructions for completing this form

1.	Company. Select the Lewis Marine Group company from the list. In most cases, this has already been completed for you.
2.	Location. Select the Lewis Marine Group location that your company conducts their business at. For example, if your company is located in New York state, most likely you are a customer of Lewis Marine Greenport.
3.	Company phone number. Enter the phone number of your company.
4.	Account number. This is your customer number with Lewis Marine. This number can be found on all Lewis Marine invoices and statements that your company receives. For example: A2012345.
5.	Enter your first and last name. This is used to "personalize" all email correspndence.
6.	Email address. Enter your email address. This email address will be used to communicate to you when your online user id and account has been established. It is also used should you ever forget your password.
7.	You may select your own user id that will be used to logon to the Lewis Marine website. Your user id can be anything you want. User ids are case-sensitive and must be a minimum of 5 letters and/or numbers.
8.	Password. You select your own password to be used when logging in with your user id. The password can be anything you want. Passwords are case-sensitive and must be a minimum of 5 letters and/or numbers.
